Referral Appointments

Ext. 227 Ė Drs. Dobbin/Saeed/Lum/Dickinson

Ext. 325 Ė Drs. Krass/Guise/Morin

We will contact patients either by phone or mail.

By phone Ė if your appointment is within two to three weeks we will call you.  All other appointments will be mailed out in due time.

By mail Ė Your appointment is usually 4+ weeks or longer.  It will include instructions on what you need to take with you to this appointment.

All pertinent information will also be forwarded to the physician who you will be seeing.  If you need to cancel or reschedule a specialistís appointment, please contact their office directly.  Most doctorís offices require at least 48 hours notice.

Diagnostic Imaging Ė CTís/MRIís/Mammograms/Ultrasound & X-rays, etc.

We will contact you directly by phone unless the appointment is 4 weeks or more away.  All instructions for the type of test will be given to you when you receive the appointment.  Priority and urgency of the appointment is determined by your family physician. 

 

Prescriptions

You must allow a 24-48 hour period for renewing all prescriptions as per the office policy created by the physicians of Niagara Health Services.  For renewals on a medication that has been previously prescribed, you can contact your pharmacy directly and they will fax the request to us. 

If you have been given samples and wish to continue on that medication you can call the office directly and we will request a prescription from your family physician to be sent to your pharmacy.

In the event you decide to switch pharmacies (moved, etc.) please call the office and request that your prescription be sent to your pharmacy of choice.

 

Narcotics/Controlled Drugs

These types of medications will only be renewed by your family physician.  If he/she is not in the office, it will be reviewed the following day.  If your family physician is on vacation or it is a weekend, another physician will review the request for repeat and may not refill the entire prescription.  It is ideal to work out a contract with your family physician.

 

Limited Use (LUís)

If a pharmacy requires a limited use form for a drug that has been prescribed, they are to request this from our office.

 

Section 8ís

Section 8 is a letter to the Drug Programs Branch which is part of the MoH to request coverage of certain drugs.  The Business Office deals with the renewal and new requests.  They are valid for one year and it is up to the patient to contact our Business Office a few months prior to the expiry.
