Ministry of Health and Long Term Care Diabetes Team

The Niagara Medical Group Family Health Team Ministry of Health and Long-Term Care (MOHLTC) Diabetes education program strives to promote diabetes self care by providing education and support to adults with type 2 diabetes and their families.

The diabetes education program helps people with diabetes learn to make healthy lifestyle choices, strives to reduce the risk of and delay the progression of complications of diabetes.

Individual and follow up assessment appointments with nurse and dietitian; as well as, group classes to learn more about diabetes.

All people with type 2 diabetes and their families within the region are welcome to attend the program.  A physician referral is needed (downloadable from website) to take to your family physician and be faxed to the diabetes program.  Most people with newly diagnosed type 2 diabetes are seen within 2 weeks.  Individuals using insulin or needing to start insulin are seen more quickly, depending on need.
